Hepatocellular carcinoma (HCC) surveillance is key to early diagnosis and timely curative treatment. Current strategies are suboptimal with many patients presenting with advanced HCC. This randomized controlled trial compared the combined use of serum biomarker (BM) measurements and abdominal ultrasound (US) with US alone for the diagnosis of early-stage HCC.

Postprocedural pancreatitis is the most common adverse event (AE) associated with endoscopic retrograde cholangiopancreatography (ERCP). Endoscopic ultrasound-guided choledochoduodenostomy (EUS-CDS) using a lumen-apposing metal stent is emerging as a promising approach for managing malignant distal biliary obstruction, offering the potential to lower the risk of postprocedural pancreatitis.

The role of low-grade-inflammation in Irritable Bowel Syndrome (IBS) a common disorder of gut-brain interaction is unclear. Association with symptom severity, as well as effects of first line treatments are understudied. In a large cohort of primary care IBS patients (DOMINO trial), a FODMAP-lowering diet application was superior in improving symptoms compared to standard medical therapy (otilonium bromide, OB). We investigated the level of pro-inflammatory markers in this IBS cohort and its changes by the DOMINO application and OB.

The risk of hypoxemia is increased in elderly patients who are sedated during gastroscopy. This can be attributed to the physiological changes associated with aging, such as reduced oxygen reserves and an increased sensitivity to sedatives. This study aimed to evaluate whether an anesthetic nasal mask is more effective than a traditional nasal cannula at preventing hypoxemia during gastroscopy in elderly patients.
5. Combined water exchange and cap-assisted colonoscopy reduces cecal intubation time and patient discomfort in patients with unsedated colonoscopy: a randomized controlled trial.
Colonoscopic insertion remains technically challenging. While cap-assisted colonoscopy (CAC) reduces the cecal intubation time (CIT), sigmoid colon water exchange (SWE) enhances linear scope advancement. We hypothesized that combined water exchange and cap-assisted colonoscopy (SWC) would optimize the CIT and improve patient tolerance during unsedated colonoscopy.

Endoscopic submucosal dissection (ESD) is a minimally invasive therapy for early gastrointestinal neoplasms, but patients may experience reduced functional capacity and delayed recovery post-procedure. Prehabilitation, has shown benefits in major surgery by improving physical fitness and recovery. We hypothesized that a structured prehabilitation regimen before ESD would enhance postoperative functional recovery and patient outcomes compared to standard care.

Palliative biliary stenting is the principal treatment for unresectable perihilar cholangiocarcinoma (pCCA) patients with jaundice. Endobiliary radiofrequency ablation (EBRFA) is a novel treatment used in combination with biliary stenting for CCA, aiming to prolong stent patency and patient survival.
