Avascular necrosis (AVN) of the hip is a disease characterized by vascular disruptions, where 67% of untreated cases may lead to the collapse of the femoral head. None of the current approaches, such as core decompression (CD), vascularized bone grafting, osteotomy, tissue implantation, and other methods, have been proven fully effective in delaying the progression of osteonecrosis. Recent findings indicate that bone marrow stem cells (BMSCs) have significant potential to regenerate necrotic tissue and prevent the progression of AVN in the hip. This study aims to evaluate the efficacy, side effects, treatment failure rate, most effective treatment stage of AVN hip, and application technique to treat AVN hip.

Nonalcoholic fatty liver disease (NAFLD) is a global chronic health challenge, demanding the development of innovative therapeutic strategies. Mesenchymal stromal cell-derived extracellular vesicles (MSC-EVs) have emerged as a promising therapeutic approach for NAFLD; however, current evidence is limited to preclinical studies. This systematic review and meta-analysis assessed the therapeutic efficacy of MSC-EVs in rodent models of NAFLD and its progressive form, nonalcoholic steatohepatitis (NASH). By synthesizing preclinical data, we aim to establish a robust evidence base that can guide future clinical trials and optimize MSC-EV-based therapies.

AIM Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is an important treatment for blood disease, and ocular graft-versus-host disease (oGVHD) is a common complication that significantly affects the quality of life of patients. Currently, the risk factors for oGVHD are still controversial.Methods To provide a scientific foundation for the prevention of oGVHD in patients undergoing allo-HSCT, studies on the factors influencing the development of oGVHD were searched in PubMed, Embase, Web of Science, Sino Med, the Cochrane Library, CIKI, the Wanfang Database, and the VIP Database from database construction to May 2024.Results Seventeen studies included 4,501 patients who received allo-HSCT, of which 1,526 were diagnosed with oGVHD, involving 22 factors. The overall prevalence of oGVHD was 37.8%.[95% CI (0.294, 0.463)].The prevalence of oGVHD based on the diagnostic criteria recommended by the National Institutes of Health was 46.7% [95% CI (0.390, 0.545)], that of the International Chronic oGVHD Group was 33.7% [95% CI (0.167, 0.506)], and that of self-defined diagnostic criteria was 32.0% [95% CI (0.184, 0.457)]. According to the meta-analysis, elderly patients [OR=1.10, 95% CI (1.00, 1.20)], female donors [OR=1.48, 95% CI (1.20, 1.83)], matched-relative donors (MRD) [OR=1.50, 95% CI (1.22, 1.83)], peripheral hematopoietic stem cells (PBSCs) [OR=1.81, 95% CI (1.38, 2.39)], acute graft-versus-host disease (aGVHD) [OR=1.74, 95% CI (1.29, 2.35)], chronic graft-versus-host disease (cGVHD) [OR=3.04, 95% CI (1.82, 5.08)], oral [OR=13.83, 95% CI (5.09, 37.56)] and skin graft-versus-host disease (GVHD) [OR=5.55, 95% CI (2.41,12.79)] were risk factors for the development of oGVHD (P < 0.0 5).Conclusion According to our systematic review and meta-analysis, the factors listed above are associated with oGVHD and can serve as early warning signs for clinicians in identifying high-risk populations eligible for early intervention and treatment.

Erectile dysfunction (ED), a prevalent male sexual disorder, severely impacts quality of life. Extracellular vesicles (EVs), natural nanoparticles (30-200 nm) secreted by stem cells, represent a novel nanomedicine platform for ED treatment due to their ability to encapsulate bioactive cargo (e.g., miRNAs, proteins) and target damaged tissues. Stem cell-derived extracellular vesicles (SC-EVs) have emerged as a promising therapeutic strategy for multiple diseases. This meta-analysis evaluates the therapeutic efficacy of SC-EVs in rat ED models and explores their translational potential.

Mesenchymal stromal cells (MSCs) have been under clinical investigation for the treatment of cerebral palsy (CP) for over a decade. However, the field has been limited by study heterogeneity and variable reports of efficacy. We conducted a scoping review of published and registered reports of MSC treatment for CP, with meta-analysis of Gross Motor Function Measure (GMFM) outcomes to summarize research and provide future recommendations. Thirty published reports and 10 registered trials were identified, including 1292 people with CP receiving MSCs. Most received ≥2 doses (72%) of umbilical cord tissue MSCs (75%), intrathecally (40%) or intravenously (38%), and 31% were treated via compassionate/Expanded access. MSC treatment was safe and meta-analyses demonstrated that MSCs conferred significant improvements in GMFM at 3 - (1.05 (0.19-1.92), p = 0.02), 6 - (0.97 (0.30-1.64), p = 0.005) and 12 months (0.99 (0.30-1.67), p = 0.005) post-treatment. Whilst MSCs are safe and improve GMFM outcomes in CP with large effect sizes, study and participant variability continues to confound data interpretation and limits subgroup analyses. With no published Phase 3 trials and high rates of compassionate access, the field would benefit from well-designed trials with unified outcomes. Additionally, data sharing to enable Individual Participant Data Meta-Analysis would support the determination of optimal source, route and dose to progress towards regulatory approval.

With age, hematopoietic stem cells can acquire somatic mutations in leukemogenic genes that confer a proliferative advantage in a phenomenon termed CHIP. How these mutations result in increased risk for numerous age-related diseases remains poorly understood. We conduct a multiracial meta-analysis of EWAS of CHIP in the Framingham Heart Study, Jackson Heart Study, Cardiovascular Health Study, and Atherosclerosis Risk in Communities cohorts (N = 8196) to elucidate the molecular mechanisms underlying CHIP and illuminate how these changes influence cardiovascular disease risk. We functionally validate the EWAS findings using human hematopoietic stem cell models of CHIP. We then use expression quantitative trait methylation analysis to identify transcriptomic changes associated with CHIP-associated CpGs. Causal inference analyses reveal 261 CHIP-associated CpGs associated with cardiovascular traits and all-cause mortality (FDR adjusted p-value < 0.05). Taken together, our study reports the epigenetic changes impacted by CHIP and their associations with age-related disease outcomes.

Acute-on-chronic liver failure has become a serious global health burden, which is characterized by an acute deterioration of liver function, rapidly evolving organ failure, and high short-term mortality in patients with chronic liver disease. The pathogenesis includes extensive hepatic necrosis, which is related to intense systemic inflammation and subsequently causes the inflammatory cytokine storm, resulting in portal hypertension, organ dysfunction, and organ failure. Mesenchymal stem cells can function as seed cells to remodel and repair damaged liver tissues, thus showing potential therapeutic alternatives for patients with chronic liver disease. However, standard treatment protocols for mesenchymal stem cells in acute-on-chronic liver failure patients have not been established.
