Liver fibrosis is the strongest predictor of long-term clinical outcomes in patients with metabolic dysfunction-associated steatotic liver disease MASLD. Non-invasive methods for identifying these patients are urgently needed. We aimed to investigate the link between the transforming growth factor-β induced protein Ig-H3 (BIGH3) and fibrosis in both diet-induced mouse model and patients with MASLD, and explore its potential use as a biomarker for MASLD related fibrosis.

Plentiful studies have reported that preoperative gamma-glutamyltransferase (GGT) had a strong relationship with the prognosis of hepatocellular carcinoma (HCC) patients treated with transarterial chemoembolization (TACE). Nevertheless, due to the small sample size and contradictory conclusions, the predictive function remains indefinite. To investigate the relationship between pretreatment GGT value and clinical outcomes in HCC patients undergoing TACE, a meta-analysis including 10 articles was constructed.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the leading cause of chronic liver disease globally, with significant liver fibrosis (SLF) determining clinical outcomes. Although complement activation is involved in MASLD pathogenesis, the clinical significance of circulating complement proteins (C1q, C3, C4, total activity) in aging populations remains unclear. This cross-sectional study aimed to evaluate the association between serum complement levels and MASLD/SLF in middle-aged and elderly adults.

With the development of comfortable medical care, the application of intravenous anesthesia in painless gastroscopy is becoming increasingly widespread. However, anesthetic drugs may have adverse effects on postoperative cognitive function, and the risk of postoperative cognitive dysfunction (POCD) is particularly worthy of attention in the elderly population. Our aim is to evaluate the effect of intravenous anesthesia on postoperative cognitive function (POCF) in patients undergoing painless gastroscopy, with a focus on identifying potential risks of POCD and guiding clinical anesthesia practices.

Hepatocellular carcinoma (HCC) is becoming increasingly prevalent as a non-AIDS-defining cancer closely tied to chronic HIV infection. It is associated with increased secretion of inflammatory cytokines, immune system dysfunction, and alterations in mitochondrial function. The objective of this study was to investigate the levels of cytokine secretion and mitochondrial DNA (mtDNA) deletion in people living with HIV (PLWH) compared with individuals diagnosed with HCC without HIV.
428. Awareness, attitudes, and clinical utilization of vonoprazan for Helicobacter pylori infection and gastroesophageal reflux disease among Chinese physicians: a nationwide cross-sectional study.
Vonoprazan has been introduced as an alternative to proton pump inhibitors (PPIs) for Helicobacter pylori (H. pylori) infection and gastroesophageal reflux disease (GERD). Despite regulatory approval and guideline inclusion in China, physician awareness, clinical utilization, and perceived barriers are not well described. This study aimed to assess the awareness, clinical utilization, and attitudes toward vonoprazan among Chinese physicians.

Endoscopic submucosal dissection (ESD) is a minimally invasive therapy for early gastrointestinal neoplasms, but patients may experience reduced functional capacity and delayed recovery post-procedure. Prehabilitation, has shown benefits in major surgery by improving physical fitness and recovery. We hypothesized that a structured prehabilitation regimen before ESD would enhance postoperative functional recovery and patient outcomes compared to standard care.

Screening esophagogastroduodenoscopy (EGD) is recommended for all cirrhotic patients at the time of diagnosis, however, non-invasive screening practices for varices have been emerging. CT scans have demonstrated high sensitivity in the detection of esophageal varices. The purpose of this study was to evaluate the sensitivity of CT scans in detecting esophageal and gastric varices when compared to the gold standard of EGD.

Peritoneal adhesion is a complication following abdominal surgery, observed in about 66% of cases. Adhesions in the intestines are common, and although they may not cause problems, they can sometimes lead to intestinal obstruction. In this study, zinc oxide nanoparticles coated with salicylic acid were used for the first time to prevent the formation of abdominal adhesions after surgery in rats.

Small bowel bleeding poses a significant diagnostic challenge for clinicians, and capsule endoscopy is currently considered the investigation of choice for evaluating suspected cases. However, evidence regarding the role of second-look endoscopy before capsule endoscopy is limited. This study aimed to evaluate capsule endoscopy findings after a negative second-look endoscopy in patients with suspected small bowel bleeding at a tertiary care center in Sri Lanka.
436. A socioeconomic perspective on the global burden of gastroesophageal reflux disease (1990-2021): findings from the GBD 2021.
Gastroesophageal reflux disease (GERD) constitutes a global public health challenge, afflicting individuals across nearly all age groups over the long term. However, few systematic investigations have assessed the associated disease burden from a socioeconomic perspective. METHODS: GERD data were extracted from the GBD 2021 database, encompassing data from the global level, 5 sociodemographic index (SDI) groups, and 24 GBD regions for the period of 1990-2021. Estimated annual percentage changes (EAPCs) were calculated to characterize temporal trends. Health inequality analysis, decomposition analysis and frontier analysis were employed to evaluate the disease burden.

The noninvasive ANTICIPATE-NASH models have been validated for risk prediction of clinically significant portal hypertension (CSPH) and liver-related events (LRE). Immunoglobulin G4 (IgG4)-related pancreatobiliary disease typically manifests as obstructive jaundice, complicating therapeutic choices. Glucocorticoids (GCs) are the primary treatment with many challenges, including high recurrence rates and patient tolerance variability. Endoscopic biliary drainage (EBD) is used to alleviate obstruction, but its role during treatment in IgG4-related pancreatobiliary disease is still contested. There remains a paucity of research comparing the potential benefits of combination therapy with GCs and EBD versus GCs monotherapy.

Few studies have evaluated the impact of biologic therapy on endoscopic findings and stenoses of small intestinal lesions in Crohn's disease (CD). This study aimed to compare the endoscopic effectiveness and stenotic changes induced by anti-tumor necrosis factor inhibitors (anti-TNF) and ustekinumab (UST) in patients with CD, along with histological and molecular comparisons of stenosis between the groups.
