201. Role of Repeat Core Needle Biopsy After Nondiagnostic Initial Biopsy for Soft Tissue and Bone Sarcoma: Systematic Review and Meta-Analysis.

Core needle biopsy (CNB) is the preferred diagnostic method for suspected soft tissue and bone sarcoma, but nondiagnostic results remain common. To clarify the role of repeat biopsy, we conducted a systematic review of studies reporting repeat CNB after an initial nondiagnostic CNB, searching MEDLINE, EMBASE and PubMed. Nine studies involving nearly 9757 initial CNBs were included. Meta-analysis showed a pooled diagnostic yield of 69% (95% CI: 0.564-0.819) for repeat CNB. Subgroup analyses demonstrated a significantly higher diagnostic yield in bone sarcoma (83.1%) compared with soft tissue sarcoma (48.5%). The repeat CNB rate following a nondiagnostic initial biopsy was 5.48%. This study represents the first systematic review and meta-analysis evaluating repeat CNB in this setting, and it highlights substantial variability in diagnostic yield between sarcoma subtypes. Further research focusing on repeat biopsy across specific sarcoma subgroups is warranted to guide clinical decision-making.

The cell surface protein TROP-2 is overexpressed in various solid tumors, making it an attractive therapeutic target. Datopotamab deruxtecan (Dato-DXd) is a novel antibody-drug conjugate (ADC) designed to selectively deliver cytotoxic agents to TROP-2-expressing cancer cells. It has recently been approved for treating unresectable or metastatic hormone receptor-positive, HER2-negative breast cancer. While preclinical and early phase clinical trials have shown promising efficacy, this meta-analysis aims to provide a comprehensive evaluation of the efficacy and safety of Dato-DXd in patients with advanced solid tumors by synthesizing the available clinical evidence.

Gallbladder cancer is the fifth most common gastrointestinal cancer worldwide, often diagnosed incidentally after cholecystectomy for presumed benign disease. Previous studies have reported geographic variations in the incidence of incidental gallbladder cancer (IGBC), but comprehensive pooled estimates examining multiple predictors of IGBC are lacking. The study aims to synthesize the risk factors and clinical predictors associated with IGBC in patients undergoing cholecystectomy for benign gallbladder conditions.

This study aims to systematically examine how minimally invasive thymectomy-covering both video-assisted and robotic-assisted methods-compares with open thymectomy in terms of perioperative safety and effectiveness for treating large thymomas. We thoroughly searched several databases for relevant studies, including randomized controlled trials and cohort studies, published up until October 2025. Data on perioperative factors, including surgical duration, blood loss, length of hospitalization, complications, and recurrence rates, were extracted and analyzed using random-effects models in STATA 18. Seven studies involving 12,880 patients were included. open surgery showed significantly more blood loss (WMD = 110.43, 95% CI 105.83-115.03; P < 0.05), longer hospital stays (WMD = 2.04, 95% CI 1.38-2.7; P < 0.05) compared to MIT. No notable difference in surgical duration was observed. Moreover, minimally invasive thymectomy (MIT) was associated with a reduced rate of recurrence after surgery (OR = 0.54, 95% CI 0.35-0.84; P < 0.05) as well as fewer postoperative complications (OR = 0.27, 95% CI 0.19-0.39; P < 0.05). MIT offers significant advantages over open surgery for large thymomas, including reduced blood loss, shorter hospital stays, and lower rates of complications and recurrence, though operative time was similar. Further large-scale studies are needed to confirm these findings and assess long-term efficacy.

Salivary duct carcinoma is a highly aggressive salivary gland malignancy characterized by rapid progression, early regional and distant metastasis, and poor clinical outcomes. Among the histopathological features associated with its aggressiveness, perineural invasion is of particular significance. Perineural invasion refers to the infiltration of tumor cells within or surrounding the perineural spaces and serves as an important route for tumor dissemination. In salivary duct carcinoma, the presence of perineural invasion has been identified as a negative prognostic indicator, correlating with increased risks of local recurrence, nodal involvement, and distant metastasis. Consequently, evaluation of perineural invasion is crucial in prognostication and treatment planning for patients with salivary duct carcinoma. A systematic review was conducted to evaluate the prevalence of perineural invasion in salivary duct carcinoma and to assess its impact on survival outcomes. Following PRISMA guidelines. PubMed, Scopus, Science direct and Embase were searched up to September, 2025 for studies on salivary duct carcinoma and its prognostic factors. Eligible studies were screened, data extracted, and quality assessed using the QUIPS Scale. Pooled estimates were calculated using a random-effects model. The pooled proportion of perineural invasion among salivary duct carcinoma cases was 0.54 (95 % CI: 0.45-0.63). Meta-analysis using a random-effects model demonstrated that the presence of PNI was significantly associated with poorer survival outcomes (HR = 1.64; 95 % CI: 1.01-2.68). For overall survival, PNI showed a trend towards adverse prognosis (HR = 1.67; 95 % CI: 0.80-3.48), while it emerged as a strong negative predictor for disease-free survival (HR = 3.32; 95 % CI: 1.77-6.23). Similarly, for disease-specific survival (HR = 1.48; 95 % CI: 0.23-9.48), progression-free survival (HR = 2.47), and distant recurrence (HR = 4.56), PNI was consistently associated with unfavorable outcomes, underscoring its role as a significant adverse prognostic factor in salivary duct carcinoma. Its presence is consistently associated with reduced survival and increased recurrence, emphasizing its prognostic importance in patient management.

Osteosarcoma is an aggressive bone cancer primarily affecting children and adolescents, with low survival rates in advanced stages. A systematic review and meta-analysis were conducted following PRISMA guidelines to evaluate the impact of ICIs on survival and toxicity in advanced osteosarcoma. ICIs show potential in treating advanced osteosarcoma but are associated with significant toxicity and uncertain survival benefits. Further research is needed to define their role and identify biomarkers for predicting response. Close monitoring for adverse events is essential.
